02/13/09 – results from gallbladder u/s came in and I do have gallstones, right now surgery is elective but recommended

02/04/09 – 6th post-op visit – 166.9 pounds – partial unfill due to 2 week long tightness issues, Dr. Nye wants me tested for h-pylori, gallbladder u/s and take some meds to help with gastrointestinal issues, if all tests come back okay, I go back in 4 week to start a slow fill process again

12/08/08 – emergency visit for unfill – 177.8 pounds – 0.5cc unfill

12/03/08 – 5th post-op visit – 181.5 pounds – 1cc fill

10/01/08 – 4th post-op visit – 206.2 pounds – 1.5cc fill

09/08/08 – 3rd post-op visit – 210.4 pounds – doing good, well ahead of the weight loss curve – found out I received a 1cc fill at time of surgery, won’t get another fill till my weigh loss stalls

08/13/08 – 2nd post-op visit – 219.1 pounds (38.81 BMI) – move to phase 5 – incisions are healing well – stop high blood pressure meds
- Phase 5 regular foods – anything my body will tolerate minus sodas and caffeine

08/06/08 – 2nd post-op rescheduled – move to phase 4 diet – remove steri-strips
- Phase 4 soft foods – all of phase 1, 2, and 3, plus tuna, soft veggies, tender chicken, fish, scrambled eggs, etc

08/03/08 – move to phase 3 diet
- Phase 3 pureed (mushies) – all of phase 1 and 2, plus pretty much anything you can put in the blender (think of stage 1 baby foods)

07/30/08 – 1st post-op visit – 224.4 pounds (39.68 BMI) – stitches removed – move to phase 2 diet
- Phase 2 full liquids – all of phase 1, plus yogurt, blenderized soups, thin cream of wheat, thin grits, sf pudding, etc

07/26/08 – released from hospital on phase 1 diet
- Phase 1 clear liquids – water, crystal lite, clear juice, broth, jello, sf popsicles, etc

07/25/08 – SURGERY DAY – 232 pounds (41.1 BMI) – Lap Band AP Standard 10cc band – 1cc fill

07/22/08 – original surgery date – rescheduled due to OR equipment shutdown

07/14/08 – pre-op appointments and surgery preregistration

06/27/08 – surgery APPROVAL (232 pounds/41.1 BMI – comorbids, high blood pressure and leukemia)

06/26/08 – Dr. Nye’s office submitted surgery package to Tricare

06/25/08 – PCM FINALLY put in correct referral

06/10/08 – all requirements completed, waiting on PCM to put in correct surgery referral

06/09/08 – psychological evaluation
